Team Ryano Portlaoise – Liability Waiver & Media Consent

By completing and signing this form, I understand that martial arts and fitness activities are contact and physical activities, and that participation in training and events at Team Ryano Portlaoise involves inherent risks of injury, including serious injury or death. I acknowledge and accept full responsibility for any risks, injuries, or damages that may occur as a result of my (or my child’s) participation.

In consideration of participation in Team Ryano Portlaoise activities, I, on behalf of myself (or my child), my heirs, executors, and administrators, waive and release all rights and claims for damages against Team Ryano Portlaoise, its coaches, staff, and affiliates. I agree not to pursue legal action for any injury, loss, or damage arising from participation, whether caused in whole or in part by negligence.

I understand the physical demands of training and confirm that I (or my child) am physically fit to participate. I confirm that I have consulted a medical professional where necessary and that there are no known conditions that would prevent safe participation.

I understand that all training is for fitness, education, and self-development purposes only, and that Team Ryano Portlaoise does not promote violence under any circumstances. I agree to follow all safety instructions and guidelines given by coaches.

I give permission for Team Ryano Portlaoise to take and use photographs and/or video footage of me (or my child) during training and events for promotional purposes, including social media and website use. I understand that images will be used responsibly, with consideration for safeguarding, and that efforts will be made to protect the identity of young members.

I understand that Team Ryano Portlaoise may publish content on its website and social media platforms to promote activities, and that all content is managed responsibly in line with safeguarding and acceptable use policies.
